Grades 3-8 CYO Athletics provides an exceptional opportunity for your child to learn new skills, have fun, be with his/her friends, and be physically active. Before any child can partici-pate in a sport at St. Vincent, a CYO Athletic Pre-Participation Form, with a DOCTOR’S SIGNATURE, is required. There is only ONE form that is acceptable.
